Are you passionate about patient care and improving liver health outcomes? Join our team at UHSussex as a Hepatology Surveillance Navigator! We are offering two exciting new Band 4 roles to support the delivery of a robust surveillance pathway for Hepatocellular Carcinoma (HCC) for local patients, in collaboration with the Surrey and Sussex Cancer Alliance (SSCA). One post will be based in Brighton and Hove at the Royal Sussex County Hospital, while the other will cover Worthing and Chichester. These fixed-term posts are funded for two years and will play a crucial role in early cancer detection for patients with cirrhosis.
Location:
1. One role based at Royal Sussex County Hospital, Brighton and Hove
2. One role covering Worthing and Chichester Monthly attendance at the Royal Sussex County Hospital for team meetings and training will be required.

Main duties of the job
As a Hepatology Surveillance Navigator, you will work closely with the Hepatology CNS team and Consultants, supporting the coordination and monitoring of surveillance pathways for patients at risk of developing Hepatocellular Carcinoma. This includes ensuring timely patient engagement in surveillance protocols, tracking key milestones in patient care, and maintaining surveillance databases.

Detailed job description and main responsibilities
6. Support scheduling of liver surveillance clinics and patient assessments.
7. Track patient engagement, ensuring timely access to surveillance scans and diagnostics.
8. Maintain and populate the liver surveillance database, ensuring data accuracy and preparing reports for NHS E.
9. Collaborate with healthcare teams to fast-track patients with abnormal results into appropriate care pathways.
10. Provide direct patient support, including appointment reminders and navigation through the healthcare system.
11. Work closely with multidisciplinary teams (MDT) to ensure streamlined patient care across sites.
12. Ensure all patient records are tracked, stored securely, and transferred between trusts as required.
13. Contribute to audit processes, service development, and ongoing improvement of the surveillance pathway.
Data Collection and Analysis:
14. Collect and update patient data, ensuring accuracy for reporting and service improvement.
15. Support the analysis of target surveillance breaches and escalate any issues to management.

18. Strong administrative experience
19. Experience liaising with patients/members of the public
