Overview
This is a fixed term/secondment post until 20/03/2027 to cover maternity leave. If you are a Hywel Dda University Health Board employee and applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application. An exciting opportunity has arisen for an experienced Occupational Therapist to join the front of house Transfer of Care and Liaison Service (TOCALS) team, at Glangwili General Hospital.
The TOCALS team is made up of proactive Occupational Therapists and Physiotherapists who work closely with medics, nursing, discharge liaison nurses, social workers, brokers, intermediate care and the wider community and third sector teams, striving to deliver the right support, at the right time in the right place as close to home as possible.
The ethos of this team is home first; they have a successful rate of turning patients around at front door and preventing admissions to hospital where it is not needed. Where identified, they will outreach and follow up a person home to prevent a further admission to hospital.

Role and Responsibilities
* As a versatile postgraduate, you will need to have strong experience of managing a complex caseload, complex manual handling, and a good understanding of current health and social care policies.
* You will need the ability and skills to work autonomously and alongside members of the team in a managerial and clinical role.
* The successful applicant will provide clinical leadership, specialist expertise and supervision to Occupational Therapy staff, Therapy Assistant Practitioners, Rehabilitation Assistants/Frailty Support Workers, and students.
* You will network with acute and community colleagues to coordinate a responsive, seamless service across pathways and sectors. You will have organisational ability and a commitment to service development by developing the evidence base for evaluation through audit, clinical effectiveness and research activity.
Qualifications and Skills
* Strong experience of managing a complex caseload and complex manual handling.
* Good understanding of current health and social care policies.
* Ability to work autonomously and with team members in managerial and clinical roles.
* Excellent interpersonal skills to collaborate with colleagues across pathways and sectors.
* Welsh language ability is desirable; applications from English and/or Welsh speakers are welcome.
