The Role
We’re seeking an experienced and compassionate Complex Care Nurse to join our community healthcare team, supporting both children and adults with complex health needs in their homes and local settings.
You’ll play a vital role in clinical assessments, care planning, and staff support, ensuring every client receives safe, high‑quality, person‑centred care.
What You’ll Do
* Carry out comprehensive assessments for clients with complex needs (tracheostomy, ventilation, PEG feeding, seizure management).
* Develop and review individualised care plans in line with best practice and CQC standards.
* Deliver hands‑on clinical care and provide training to healthcare assistants in the community.
* Work closely with families, GPs, therapists, and other professionals to coordinate care.
* Monitor and audit care quality, ensuring compliance with safeguarding and governance policies.
* Participate in the on‑call rota, offering clinical advice and escalation support when needed.
What We’re Looking For
* Registered Nurse (RGN / RSCN / RNLD) with active NMC PIN
* Minimum 2 years’ post‑registration experience in complex care, community, or acute nursing
* Proven skills in at least one of the following:
* Tracheostomy care
* Ventilation management
* PEG / enteral feeding
* Catheterisation and bowel management
* Seizure management and medication administration
* Strong leadership, risk assessment, and documentation skills
* Excellent communication and interpersonal abilities
* Full UK driving licence and own vehicle
Ideally, You’ll Also Have
* Acute experience with both children and adults in HDU/ITU settings
* Confidence in tracheostomy care, ventilation, seizure management, and chest management
* Experience in paediatric complex care
* Previous homecare or community nursing experience
* Clinical training or mentorship background
