The Transfer of Care Hub at Colchester Hospital is looking to recruit a Senior Case Manager to join our developing team.

Location is at our Community Hospitals, working between Clacton and Harwich, with support at Colchester Hospital when requested.

Must be a car driver.


Role Overview

The aim of the discharge team is to maximise personal independence and enable patients to return home as quickly as possible in a supported manner. You will be responsible for early identification of complex patients and liaising with the multi-disciplinary team to provide appropriate support, promoting a home first ethos.

You will carry out duties supporting the Transfer of Care Hub in delivering high-quality care and effective discharge planning. The focus is to manage and direct, under supervision of a Lead Case Manager, the gathering of detailed information to support discharge plans over a seven-day period. Clinical skills are not required, but you will be patient-facing, gathering information and liaising with families/next of kin for robust assessments.

This is a fixed-term/secondment position until 31st December 2025. Internal applicants currently employed by ESNEFT will be offered this role on a secondment basis only.


Key Responsibilities

1. Maintain up-to-date knowledge of discharge pathways, including the DOH Hospital Discharge policy and the Discharge to Assess model.
2. Advocate for patients and their families.
3. Manage and prioritise your own caseload while supporting fellow Case Managers to ensure timely discharges.
4. Support discharge planning and system flow within a seven-day service to promote patient independence.
5. Maintain accurate patient records and ensure electronic records are current, reflecting the Criteria to Reside.
6. Understand the Mental Capacity Act and its implications for discharge.
7. Initiate discharge planning promptly after patient admission.
8. Establish effective communication with stakeholders, including Transfer of Care Hub, ward staff, pharmacy, therapy services, doctors, nursing, care homes, and voluntary services.
