Exciting opportunity for an experienced Practice Nurse to join our friendly general practice. You will undertake all aspects of the practice nurse role, including cervical screening, baby immunisations and chronic disease management.
We are a practice serving approx 5000 patients.
You will be joining a team which comprises GPs, Pharmacist and a Physician Assistant, Mental health nurse and physio's. The practice provides patient services to a richly varied, multicultural population.
Experience in General Practice is essential.
Main Duties of the Job
* Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
* Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems (We use EMIS web as our main clinical system) for input of information for recall and screening purposes.
* Providing specialist care for registered patients in the following key areas: Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraception, Baby immunisations.
* Providing routine treatment in general clinics, including wound management and blood tests.
* Health promotion, chronic disease management, health prevention and supporting the management team in reviewing clinical policies and procedures.

Person Specification
Qualifications
* UK Registered nurse with NMC; Minimum 2 years post registration; Experience working in General Practice setting; Demonstrate an understanding of audit; Cervical Cytology; Child and Adult immunisations; Care of Long Term Conditions (LTCs); Experienced in Wound care.
Desirable
* Qualified in Phlebotomy

* Excellent communication skills (written and oral)
* Strong IT skills; Clear, polite telephone manner; Competent in the use of Office and Outlook; IT skills (EMIS, MS Office, Internet)
* Interpersonal skills; Time Management and timekeeping; Knowledge of clinical governance; Ability to work as a team member and autonomously; Good interpersonal skills; Ability to follow clinical policy and procedure.
