Job Title: Discharge to Assess - Care Assistant Location: Taunton & Surrounding Areas Pay Rate: £14 per hour & paid travel mileage Shift Times: Morning: 7am – 3pm Evening: 2pm – 10pm Split: 7am – 2pm & 4pm – 10pm (with 2-hour break) We are seeking a dedicated Discharge to Access (D2A) Care Assistant to join our clients team in Somerset and the surrounding areas and therefore access to your own transport is essential. Our client is committed to helping customers lead independent lifestyles post-diagnosis or health condition. As a D2A Care Assistant, you will play a pivotal role in assisting customers with their rehabilitation needs in the comfort of their own homes. Key Responsibilities: Provide safe, reliable, and compassionate support tailored to each customer's individual rehabilitation programme. Collaborate with the care team, hospital discharge team, and occupational therapist to assess and meet the rehabilitation needs of customers. Respect and promote the dignity of each customer, ensuring care is delivered with informed consent. Monitor and report customer progress towards achieving rehabilitation goals in a professional and timely manner. Ensure clear communication with cluster groups of fellow professionals to aid customers' rehabilitation. What We're Looking For: Right to work in the UK (we are unable to provide sponsorship). Full UK driving licence and access to a car. Class 1 business insurance (or willingness to obtain). A caring, reliable, and respectful nature. Willingness to undertake full training. What You’ll Get £14.00 per hour. Paid travel time at £12.21/hr. Mileage allowance of 35p per mile. Full training provided, ensuring you are well-equipped to deliver excellent care.
