The role is ideal for an experienced practitioner who is confident in complex case management and committed to high‑quality, person‑centred practice. A key focus of the role is completing Care Act Reviews and ensuring individuals receive the right level of support. You'll play a central part in maximising Continuing Health Care (CHC) and other health funding, working collaboratively with health partners to secure appropriate packages of care. About You Qualified Social Worker with Social Work England registration, 2 years permanent experience.
