We are looking for anAdvanced Nurse Practitioner to join our friendly experienced team at ourSparcells site. The role will involve working independently as anautonomous practitioner providing clinical assessment, diagnosis and treatmentfor our adult and paediatric patients with undiagnosed and existingconditions. You will be working alongside our clinical team of GPs,Nurses, Pharmacists, Physiotherapists, Health Care Assistants and otherassociated roles to provide safe, holistic care to our patients. This willinclude face to face and remote appointments. You will be required towork within your professional competence and boundaries to provide both urgentand planned care to patients. You may also be required to complete ward visits to a local care home.

Job Summary/Main Purpose
The post holder willwork autonomously to provide comprehensive healthcare that is sensitive to theneeds of the population. They will takea leading role in managing the clinical services provided by the practice,ensuring that they meet the needs of our patients and fulfill the terms of ourcontracts.
SPECIFIC RESPONSIBILITIES
Clinical
See patients thatpresent with previously undifferentiated, undiagnosed problems, takingresponsibility for their health assessment and physical examination, thediagnosis of their condition and its management or referral to another memberof the multi-disciplinary team deemed appropriate by the practitioner withintheir professional competencies.
Make differentialdiagnosis using decision making and problem solving skills.
Make professionallyautonomous decisions for which they are accountable.
Order necessaryinvestigations and provides treatment and care, both individually, as part of ateam and through direct referral to other primary and secondary agencies.
Plan,implement and evaluate the chronic disease management programmes in line withcurrent evidence based best practice and the GMS contract, reducing unnecessaryhospital admissions and promote the preventative aspect of nursing practice.
Prescribeappropriately as an Independent Nurse Prescriber, using the practice formulary
Screenspatients for disease risk factors and early signs of illness following NICEguidelines.
Createpatient care plans to include health promotion, health protection and diseaseprevention as appropriate.
Torecord information in patient records accurately and in linewith current professional guidelines.
Management
Work to the NMC Code ofProfessional Conduct.
Provide leadership and supportto the clinical team at the practice.
Actively monitorprogress in the delivery of contracted services.
Makea significant contribution to the development and direction of the practice.
Participate inmulti-disciplinary meetings.
Participate in staffappraisal.
Promotecommunication and provide learning opportunities for other staff.
Education, Training and Development
Hold a Masters degreein Advanced Practice
Develop new skills byundertaking training as required.
Maintain current skillsby study and attending update training.
To maintain aprofessional portfolio and actively keep up to date with the latestdevelopments and guidelines for best practice and disseminate this informationaccordingly.
Take part in theappraisal process.
Revalidate in line withnational guidelines.
Encourage, facilitateand participate in clinical supervision within the practice.
To be a resource for adviceand guidance for other members of the team.
Research
Monitor research inorder to improve quality care and disseminate research finding.
Ensure evidence basedpractice personally and throughout the clinical team.

Experience
* One years experience post Nurse Practitioner Degree.
* Experience of multi-disciplinary working.
* Able to demonstrate high level of personal professional autonomy and accountability.
* Work using advanced clinical skills.
Clinical knowledge and skills
* Competent clinical skills with a knowledge of chronic disease management.
* Able to use analytical skills in practice.
* Excellent interpersonal and organisation skills.
* IT experience.
* Clear understanding of legal issues, professional accountability and responsibility.
* Experience of SystmOne
Qualifications
* RGN
* Recognised 3 year Nurse Practitioner Qualification to include autonomous practice.
* Chronic Disease Management - Asthma, Diabetes, Chronic Obstructive Pulmonary Disease, Coronary Heart Disease, Stroke, Family Planning
Other experience
* Excellent written and verbal communication skills.
* Experience in liaising with General Practitioners and other Health Care Professionals.
* Experience in managing RGNs and Health Care Assistants.
* Ability to be flexible and adaptable to change.
* Understanding of clinical governance.
* Ability to work under pressure.
* Able to manage time effectively.
