Overview
Surrey Downs Heath & Care - Epsom PCN. This is an exciting opportunity for a highly motivated and enthusiastic Healthcare Support Worker to work within the community nursing team at Leatherhead PCN. You will provide care and support to patients within the community setting, undertaking a range of delegated tasks. The post-holder will be supported to develop skills and abilities to enhance their career and provide appropriate care to patients.

This is a 7-day service from 8:30am to 6:30pm; staff are required to work flexibly across the full week and no more than one weekend in four.
Main duties and responsibilities
* Clinical: Coordinate and participate in specialist care (e.g., wound care, Doppler testing, phlebotomy, bladder screening) under supervision of Registered Nurses; undertake diagnostic tests on request from District Nurses, Community Matron or GPs (including home visiting or coordinating repeat testing for residents in residential homes); undertake clinical interventions as delegated by a senior team member.
* Health Promotion: Participate in the assessment and implementation of holistic care programmes; support, educate and inform family/carers where appropriate; assist in promoting health and wellbeing of patients.
* Administration and Clerical Duties: Maintain accurate records; take and record messages; manage stationery and documentation supplies; maintain data entry and record keeping (e.g., mileage, annual leave) on staff databases.
* Team Working: Work as part of the team to deliver procedures and duties to agreed standards; assist with clerical and administrative duties; maintain regular contact with the senior team member.
* Other Duties: Preserve patient privacy and dignity; liaise with other professionals and agencies; follow safeguarding procedures; participate in governance and risk management strategies; undertake duties reasonably required within the post’s responsibilities.
Qualifications and Requirements
* Essential
NVQ Level 3 or an equivalent qualification or course; Full UK driving licence.
Skills
* Ability to provide quality, patient-centered care; strong written and verbal communication; ability to work effectively as part of a team; access to a vehicle for work.
Experience
* Essential: Experience of working directly in a care coordinator role, adult health and social care, learning support or public health/health improvement.
* Desirable: Significant administrative and clinical experience.
