Intermediate Care Practitioner

Closing date: 20 May 2026.

Role summary: ABand 7 Frailty Nurse providing advanced clinical assessment, complex decision‑making, and proactive case‑management for adults living with frailty across community settings. The role focuses on preventing avoidable hospital admissions through timely intervention, holistic assessment, and coordination of multidisciplinary care. Working autonomously, the postholder leads frailty pathways, supports Urgent Community Response activity, and delivers person‑centred care in patient homes.


Key Responsibilities

* Undertake advanced physical examinations and clinical decision‑making.
* Diagnose, plan, implement, and evaluate personalised care plans.
* Manage complex, multimorbid patients with fluctuating needs.
* Prescribe or recommend medications (as non‑medical prescriber).
* Identify and manage acute deterioration, including escalation where required.
* Deliver urgent community response (UCR) interventions to prevent hospital admission.
* Provide clinical triage in the Single Point of Access centre on a rotational basis.
* Link with existing services to prevent hospital admission and facilitate early discharge.
* Develop partnerships and joint working within the wider local health and social care economies.
* Work collaboratively across organisational boundaries to support effective, coordinated provision of health and social care services.
* Conduct comprehensive healthcare assessments, including medical history, physical examination, medication review, functional/cognitive assessment and referrals for diagnostic tests.
* Maintain accurate clinical records in line with professional, national and local standards.
* Implement best practice based on evidence and clinical knowledge; monitor and manage plans of care in collaboration with MDT.
* Coordinate care and treatment to avoid fragmentation, duplication and delay, prioritising and coordinating multiple health needs and referrals to specialist services.
* Act as a clinical lead and mentor to team members and students; challenge professional and organisational boundaries to promote continuity of high‑quality patient‑centred care.
* Contribute to service development, data collection, and quality improvement initiatives, including audit and research.
* Ensure compliance with clinical governance, policies and procedures, including incident reporting and patient satisfaction.
* Provide education, advice, and support to professionals, patients and carers; maintain up‑to‑date knowledge and competence.


Required Skills & Knowledge

* Advanced clinical assessment and decision‑making skills.
* Experience with long‑term conditions, frailty, and triage of patients.
* Strong communication, negotiation and influencing skills.
* Capacity to organise and prioritise complex caseloads.
* Knowledge of practice standards, clinical governance and safeguarding procedures.
* Experience of working across multidisciplinary teams and in community settings.
* Ability to use data and information tools for monitoring and reporting service outcomes.


Qualifications

* Registered Nurse or Allied Health Professional with current NMC or HCPC registration.
* Degree or Master’s level clinical/advanced practice qualification.
* Prescribing qualification at V300 level, or willingness to undertake.
* Evidence of post‑registration study on management of COPD, Diabetes or Coronary Heart Disease.
* Mentorship course/qualification or equivalent experience.


Experience

* Extended post‑registration experience and evidence of enhanced clinical practice.
* Management or clinical leadership experience.
* Experience working in community settings and multi‑agency environments.
* Work autonomously and under pressure.
* Experience of participating in research and audit.


Sponsorship & Working Eligibility

* Confirmation of Right to Work status; share‑codes will be checked where applicable.


Other Requirements

* Flexibility to work over 7 days on rota, 8 am‑8 pm.
* Ability to travel independently across the Trust.
* Willingness to undertake further study if required.
* Skills in moving and handling, venepuncture, palliative/end‑of‑life care.
