Job Title: Adult Social Worker - CMHT Location: Blackpool (Hybrid) Pay Rate: £30.66 per hour Job Description: Liquid Personnel is recruiting an Adult Social Worker for its client's CMHT department in Blackpool. What will your responsibilities be? Conducting Mental Health Assessments Carry out comprehensive assessments of individuals experiencing mental health difficulties, including risk assessments and safeguarding evaluations. Collaborate with service users, families, and multidisciplinary teams to develop care plans that promote recovery and independence. Care Coordination and Case Management Act as a Care Coordinator under the Care Programme Approach (CPA), ensuring timely reviews, documentation, and coordination of services. Liaise with psychiatrists, psychologists, nurses, and external agencies to ensure holistic support for clients. Crisis Intervention and Safeguarding Respond to mental health crises, including arranging Mental Health Act assessments when necessary. Identify and manage safeguarding concerns, making referrals and attending strategy meetings as appropriate.
