We are working on behalf of a well-established, high-quality home-care provider in Essex who are seeking an organised, proactive, and compassionateCare Coordinatorto join their growing team.This is an excellent opportunity for someone with strong care-sector experience who is ready to step into a pivotal coordination and leadership role supporting both staff and service users.As a Care Coordinator, youll play a crucial part in ensuring that individuals receiving care experience dignity, independence, and consistency throughout their support. You will work closely with the Registered Manager to deliver a well-structured, compliant and person-centred service.
Key Responsibilities
Care & Service Quality
1. Coordinate high-quality home-care packages that meet regulatory requirements and company standards.
2. Assess needs, develop personalised care & support plans, and ensure risk assessments remain current.
3. Maintain accurate digital records and ensure documentation meets CQC expectations.
4. Manage new referrals, accept care packages, and support with fee discussions when required.
