Community Support Workers work with our multidisciplinary team in Penwith (Penzance, Hayle, Marazion, St Ives, and surrounding areas), delivering reablement as part of a collaboration between community, acute, and social care providers.
The reablement approach helps people to return to or stay in their homes. We seek candidates who can motivate individuals to aid their recovery, maximize independence, achieve their goals, and get home from hospital sooner.
You will be part of a dedicated team and receive bespoke training to support your development. Good communication skills and a positive attitude towards older people, including those with communication difficulties or complex health needs such as frailty, are essential.
The team provides a 7-day service to community patients.
This community-based role requires frequent travel across Penwith, including areas not always accessible by public transport.
A driving license and access to a car are essential for this position.
Main Duties of the Job
The primary purpose of this role is to provide a versatile and responsive service to optimize opportunities for patients' reablement in the community.
Community reablement workers form part of an Integrated Care Pathway aimed at preventing hospital admissions and supporting discharges.
The post involves delivering care packages to patients in their homes, including residential and nursing homes, with mileage allowances for travel in your own vehicle.
Flexibility in working patterns is required to meet service needs, with rostered shifts accordingly.

Person Specification
Education, Qualifications & Experience
* Essential: NVQ Level 3 in care or equivalent experience
