We have contracts with local authorities and private contracts for customers with personal budgets to provide specific home care services to people with mental health diagnosis, physical health conditions, learning disability and low- or high-level health care needs, to enable recovery and minimise hospital admissions.
About the Role
Are you an experienced and compassionate individual looking for a new challenge in the health and social care sector?
We have an exciting opportunity for a Care Coordinator to join our team and play a vital role in supporting our business with new care packages and overall care coordination.
This dynamic role offers a blend of office-based and community-based work. Your primary hours will be Monday to Friday, 8am to 4pm, but we're looking for someone who can be flexible. You'll need to support care delivery for new packages that may fall outside these hours, and there will be occasional weekend work as needed.
As part of our senior team, you'll contribute to our out-of-hours on-call rota, for which an enhanced weekly payment will be provided.
Key Responsibilities
* To organise the care team whilst considering the changing needs of customers
* To compile weekly care schedules ensuring that all customer calls are covered with suitably trained and experienced staff
* To react to changing circumstances such as staff absence and ensure care calls are still covered
* To maintain electronic records and hard copy information such as care plans, risk assessments and customer review records
* To clearly set out the expectations for each shift and direct the team accordingly
* To promote dignity, respect, and choice for customers, always ensuring that the care-team work within this ethos
* To be accountable to the deputy manager; ensuring that they are kept informed of any concerns or changes to the health and wellbeing of individual Customers
* Ensure that care and support are provided for customers in a timely way upholding and improving standards of care delivery
* To ensure that customers have daily opportunities for meaningful occupation and activity
* To Liaise with Local Stakeholders such as Local Authority Brokerage Teams, Health Professionals, and social workers
* To ensure that all care staff engages in the activity and occupation programme of supporting customers involvement to enhance wellbeing and quality of life
About You
We are looking for someone with excellent people skills, who thrives in a fast-paced environment where priorities can shift quickly.
Strong IT skills are also essential, as you'll be working with an electronic care system.
As an ideal candidate, you'll hold a Level 3 Health and Social Care qualification.
You must also be a driver with access to your own car for work purposes.
