We're looking for a bright, motivated and hardworking nurse to join our energetic and supportive Integrated Discharge Team at Broomfield Hospital on a 12‑month secondment / fixed‑term basis.
In this rewarding role, you'll be central to helping patients move safely and smoothly from hospital to home or onward care. You'll assess patients who are medically optimised for discharge, identify their ongoing care needs, and coordinate timely referrals to community and system partners—ensuring every discharge is safe, effective and patient‑centred.
Strong communication is at the heart of what we do. You'll work closely with patients, families, multidisciplinary teams and wider system partners, using your clinical judgement and interpersonal skills to remove barriers to discharge and maintain effective patient flow across the hospital.
