Position:
An exciting opportunity has arisen for an Associate Practitioner to join the Hospital at Home (H@H) service currently based within Ailsa Campus.
Hours:
From 1 April 2026, the working week for NHS Agenda for Change staff in Scotland will be reduced. Full-time hours will reduce from 37 to 36 hours (pro rata for part-time staff) without loss of earnings.
Shift pattern is Monday Sunday, 08.00-2030, 365 days of the year.

UK Driving Licence/Car Ownership:
Full UK driving licence is essential to this role.
What you will do:
We areseeking to recruit a motivated & dynamic individual as a Hospital @ Home associate Practitioner to join the Hospital @ Home Service. This service delivers hospital level care to the patient in their home setting or place of care, whilst under the care of a MDT including a named Hospital Consultant.
Candidates must be driven to provide a person-centred care experience for individuals & have a thorough knowledge of frailty. Candidates should have the knowledge & skills required as part of the MDT in managing the acutely unwell adult. The successful candidates will work across professional boundaries & with both Health & social care services to achieve the best outcome for patients.

Knowledge, training and/or experience required to do the job:
Candidates will deliver high quality care in accordance with guidance given by registered staff & should be educated to SCQF Level 8 or equivalent and have the relevant experience to enable them to use specialist technical & practical skills to deliver flexible, person centred care under indirect supervision of the registered staff, this may include cannulation & venepuncture, ECG recording, bladder scanning, patient observations & measuring for mobility aids.
Knowledge and experience of providing direct clinical care that encompasses assessment, treatment and management of frail patients who are acutely unwell and make timely decisions regarding their management. This involves making onward appropriate consultant / clinic / rehabilitation / MDT referrals and to discharge patients directly from the service.
Treat and manage a large and delegated clinical caseload under supervision and to establish and develop skilled practice in the multidisciplinary management and care of frail patients with a range of acute conditions within homely settings.
Have the ability to assess and manage significant clinical risk within own delegated caseload this involves quick and effective identification of serious conditions/deterioration and the necessary escalation to Advanced Nurse Practitioners and other healthcare professionals, which may require onward referral.
