Key Responsibilities:
Conduct comprehensive Occupational Therapy assessments, including reablement and rehabilitation-focused evaluations.
Develop and implement tailored care and recovery plans that promote independence and inclusion.
Manage and prioritise a diverse caseload efficiently, ensuring each service user receives high-quality and timely support.
Regularly review progress and adapt care plans to meet evolving needs.
Collaborate with multidisciplinary professionals to ensure coordinated and person-centred care delivery.
Requirements:
Registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
Full UK driving licence and access to a vehicle (essential, due to travel requirements across South Leeds).
