Your Role
* Conduct assessments of patients in hospital, including those on Pathway 3, to plan safe and effective discharges.
* Develop and implement short- and long-term care plans for patients with complex needs, supporting independence and well-being.
* Work in partnership with multidisciplinary teams, external agencies, and community services to provide coordinated care.
* Provide guidance and support to patients, families, and carers throughout the discharge process.
* Manage complex cases, using your strong analytical, interpersonal, and decision-making skills to ensure safe and effective outcomes.
* Contribute to improving discharge pathways, processes, and care planning for complex patient needs.

What We Need
* You must hold a Social Work Qualification and be registered with SWE.
* Experience in hospital social work, discharge planning, or community care is highly desirable.
* Experience working with Pathway 3 patients, complex cases, or ongoing care needs.
* Strong ability to work in partnership with multidisciplinary teams and external agencies.
