This post is based in the West Kent area. As a Case Officer within the Short-Term Pathways Team, you will be working with people over the age of 18 with a wide range of needs. You will manage a small caseload requiring assessment, review, and arrangement of on-going support to make a real difference in achieving improved outcomes and promoting independence.
Focusing on planning ongoing care and support for adults who have recently had a hospital admission, this role is within a team that works collaboratively with health colleagues and other partnership agencies and focuses on promoting a persons independence.
Whilst mainly working from home at the current time, a fixed working base will be discussed at interview.
The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential this covers the whole of West Kent area.
This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
