This is an exciting opportunity to be part of a team putting people first to make sure they get the right support at the right time, ensuring the same day discharge for those individuals who are clinically ready to return home. As part of our Home Assessment Team, you will work alongside Health & Social Care and Acute colleagues whose focus is prevention of delays in hospital discharge arrangements. Working alongside the individual and their families and other support networks ensuring good quality support in those first days after returning home. Within the North Lanarkshire Partnership our ambition is beyond same day hospital discharge, but prevention of admission at the front door of the hospitals and within community, where response to crisis situations can prevent hospital admission.
We have experienced an unprecedented demand for services, and we are currently recruiting for people to join us and help ensure we continue to deliver our high levels of performance and standards. We are looking for someone to lead, develop and motivate staff and has an excellent track record with significant post qualifying experience. You should be highly motivated and an inspirational social work practitioner, passionate and committed to driving forward change, whilst promoting innovation in a culture where people feel supported to do their jobs.
Significant post qualifying experience is essential together with a CQSW/DipSW, BA (Hons) in Social Work or equivalent. You are required to become a member of the Protecting Vulnerable Groups (PVG) Scheme prior to commencing in this role and be registered or eligible for registration with the Scottish Social Services Council (SSSC).

Friday: 08:45am - 16:15pm
