Overview
We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients own homes and residential care homes.
Responsibilities
The post involves liaising with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to this group of patients and their carers. The post holder will have a high level of knowledge in the field of community care and will have a specific interest in areas such as palliative care, long term conditions, catheterisation and wound care. You will be required to work autonomously to provide a whole person approach to the care and management for patients within this setting.
PLEASE NOTE THAT THIS POST WILL CLOSE EARLY IF A SIGNIFICANT NUMBER OF APPLICATIONS ARE RECEIVED. You will need to be flexible, adaptable & a good team player with the ability to work independently within a community setting. You will be prepared to work all shift patterns within the locality including early and late shifts, weekends and bank holidays. There may also be the need to provide cover for the out of hours District Nursing team.
Qualifications
Qualifications for the post include NVQ level 3 in health and social care. You will be required to provide original evidence of the necessary qualifications for the post at interview should you be shortlisted. You will also be required to provide original copies as part of the pre-employment check screening. The essential qualifications required for the post are outlined within the Person Specification.
Employment Details
* New Starters - Probationary Period: For any new employees commencing employment with North Tees and Hartlepool NHS Foundation Trust, you will be subject to satisfactory completion of a 6 month probationary period. Internal applications and individuals appointed to fixed term appointments of less than 6 months are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or a bank member of staff).
* Employment References: All applicants are required to provide references covering their last three years employment history including any gaps. Applicants should ensure e-mail addresses for all referees are included on their application form. Personal references are not acceptable.
* Working Hours: If the hours advertised for the post are up to full time (37.5 hours per week), applications are still welcomed from individuals who may be seeking to work less than full-time hours and all requests will be considered in line with the post being appointed to for the successful applicant.

Pre-employment and DBS
For new staff, DBS clearance is required. The DBS check will be undertaken during the pre-employment screening. The method of payment for the DBS clearance is via salary deduction from your first month's pay on commencement with the Trust.
