Overview
We are recruiting a Senior Nurse to join our team in Morley. Our service is at the heart of the NHS long‑term plan, providing more care in patients’ homes, while enabling early discharge from hospital and reducing hospital admissions. When you join us, we will work with you to develop your own personal development plan. The service runs 8 am until 8 pm every day, and we offer flexible work opportunities.
Responsibilities
* Serve as a clinical leader and oversee the delivery of high‑quality care within a Neighbourhood Team.
* Manage a clinical caseload and lead a cluster of Staff Nurses and Neighbourhood Clinical Assistants to provide excellent patient care.
* Be dynamic, innovative, passionate, and committed with experience of case management and multidisciplinary working.
* Support early discharge and reduce hospital admissions.
* Participate in case management and multidisciplinary team meetings.
Qualifications
General Requirements – Essential
* Excellent communication skills and the ability to motivate others.
General Requirements – Desirable
* Experience in community nursing and case management.
