About the Role

* As an Outreach Worker, you will play a vital role in supporting rough sleepers to leave the streets and achieve lasting independence.
">
* You will identify and engage with rough sleepers, offering consistent and ongoing support to help them access appropriate services, including accommodation.
">
* Your expertise will be essential in managing a caseload and making effective multi-agency relationships to ensure the best possible outcomes for service users.
">
">

Key Responsibilities

">
">
1. Identify and target rough sleepers, including those unwilling to initially engage, using shift patterns that reflect their habits.
">
2. Manage a caseload of service users, offering regular support and guidance to help them achieve their goals.
">
3. Assist rough sleepers to access relevant services, such as accommodation and healthcare, and make referrals to other agencies as necessary.
">
4. Maintain effective relationships with multiple stakeholders, including other professionals and community groups.
">
">

About You

">
">
* You must have a driver's license and previous experience working with vulnerable people.
">
* You should be proactive and able to work independently, as well as part of a team.
">
* You will need to participate in shift work, including evenings and weekends, to maximize contact with service users.
">
* You should be able to record accurate notes and complete forms in a timely manner.
">
* You will be required to demonstrate empathy and compassion when working with service users.
">
">
