Following a successful pilot project and the progression of the previous post-holder, we are recruiting a Community Support Worker to join the Discharge to Assess team. The team covers all of Hertfordshire (North, East, South, West). The role will work alongside a Band 6 colleague, either allocated to the East and North of the County or West and South.
South CAMHS is a multi-disciplinary team providing mental health assessment and intervention to young people aged 0-18 presenting with complex mental health difficulties.
Our team consists of psychiatry, clinical psychology, systemic therapists, child psychotherapists, clinical social workers, nursing, CAMHS practitioners and support workers.
Main duties of the job
You will support a smooth transition from hospital to care home in close partnership with the Community Psychiatric Nurse assigned to the area. The role is both clinic-based and involves significant outreach and community support. You will work flexibly to support frail service users who require assessment in our D2A beds. Experience working with individuals with mental health difficulties and complex physical health is essential.
Regular supervision and support will be provided, with opportunities to develop your skills, especially in physical health.
Job responsibilities
* Work under the supervision of a Registered Nurse while managing your own diary and caseload, reporting findings and assessment results to the Registered Nurse.
* Participate in the assessment of both physical and mental health needs; understand the relationship between physical and mental wellbeing.
* Collaborate with other agencies to deliver a systems approach ensuring service users’ needs are optimised during the Discharge to Assess period.
* Support the assessment of service users identified as having a mental health need; take and record mental state examinations and perform physical health checks (blood pressure, pulse, weight, BMI). Carry out venepuncture for required blood tests.
Person specification
Knowledge, training and experience
* Good standard of education, including Maths and English GCSE grade C or above or equivalent.
* Experience working with older people in an acute or community setting.
* Experience with older people with mental health conditions and complex physical health.
* Experience working autonomously and using your own initiative.
* Ability to manage distressing and challenging situations.
* NVQ in Health and Social Care.
Communication skills
* Excellent communication skills, verbally and in writing.
* Ability to provide feedback and discuss cases within the multi-disciplinary team.
* Excellent documentation of duties performed.
Analytical skills
* Good range of problem‑solving skills.
* Good gathering skills.
* A positive approach to working with the older generation.
* Basic understanding of the legal framework for mental health care.

Salary: £29,812 to £32,715 a year, pro rata (5% HCAS included).
