* Identifying residents’ psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those Fosters a climate, and routine that enables residents to maximize their individuality, independence and dignity.
* Establishes policies and programs and administers such programs, working with Board of Directors committees.
* Determines policies and defines scope of services to be rendered within legislative regulations for public welfare agency functioning without Board of Directors.
* Assumes responsibility for development and administration of standards and procedures related to personnel, including staff development, budget, and physical facilities.
* Interprets agency purpose and program to community.
* Establishes and maintains relationships with other agencies and organizations in community toward meeting community needs and services.
* Plans, develops, organizes, evaluates and directs the social service program of this facility.
* Involves patients and families with coordination and participation in the interdisciplinary planning process, relative to care planning, assessment functions, and to make referrals for appropriate home care services, prior to the resident’s returning to the community.
* Provides information to resident/families as to Medicare/ Medicaid, and other financial assistance programs available to the resident.
* Reviews departmental complaints and grievances and makes written reports to the Administrator of action(s) taken. Must follow Facility’s established policies.
* Assists the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies along with performance improvement projects as needed.
* Ensures that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
* Participates in regularly scheduled reviews of resident care and discharge plans.
* Meets with social services personnel, on a regularly scheduled basis, and solicit advice from inter-department supervisors concerning the operation of the social services department, assist in identifying and correcting problem areas, and/or the improvement of services.
* Recruits, hires, trains, schedules department work hours, monitor absenteeism, counsel / discipline, evaluate, check competency, and terminate social service staff as requested or necessary; complying with all state and federal regulations.
* Develops and maintains an effective orientation program that orients the new employee to the department, its policies, procedures and assigned job functions including but not limited to all staff development / education of the social work position.
* Adheres to safety and sanitation and assists the Safety Officer in developing safety standards for the reduction of potential violence in the workplace; and help eliminate tasks in the workforce that may result in bodily injury.
* Forecasts the needs of the department and is involved with the budget and planning process for the Social Services department.
* As a Resident’s Rights Advocate maintains the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information; and adhere to Resident’s rights and adhere to all HIPAA
* Investigates and follows up on any patient abuse complaints in accordance with the established policies and regulations and by professional licensure.
* Performs other work related duties as required or requested.

* Must possess a Bachelor’s Degree in Social Services
* Must have, as a minimum, two (2) years experience in a supervisory capacity in a hospital, nursing care facility, or other related medical facility.
* Must be able to read, write, speak, and understand the English language.
* Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
* Must be a supportive team member, contribute to and be an example of team work and team concept.
* Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
* Must possess the ability to make independent decisions when circumstances warrant such action.
* Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
* Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
* Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
* Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.
* A member in good standing in the National Association of Social Workers and Academy of Certified Social Workers, Inc., preferred.
* Must not pose a direct threat to the health or safety of other individuals in the workplace.
* Must be able to lift a minimum of 50 pounds and meet all other Physical and Sensory requirements of the job.

To plan, organize, develop, and direct the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. needs.
