HMP Parc In-Reach Community Psychiatric Nurse (CPN)

The post holder will be responsible for the care coordination of prisoners on caseload and Contributing to an effective secondary care mental health service within HMP Parc.

You will work under the Mental Health Measures framework to assess need and plan care and treatment for individuals who find themselves detained in custody, on remand or convicted and require secondary care mental health services .

You will work within the Mental health In-Reach team as a CPN, working closely with members of the prison health care teams, G4S operational staff, forensic teams, CMHT and probation teams.

The post holder will be expected to meet the complex needs of the identified client group, be responsible for liaison with key agencies (statutory and Voluntary) within the prison community and the wider geographical community.

Main duties of the job

Work as part of a multi-disciplinary, multi-agency team fostering good communication with colleagues of all disciplines and adhere to the Values set out by CTMUHB.

Provide / receive complex, sensitive and contentious information within a potentially hostile, antagonistic or highly emotive atmosphere in a secure setting.

Be aware of the importance of maintaining confidentiality and security of information gained during the course of their duties.

Ensure smooth transfer of care to specialist/out of area placements with appropriate local and national agencies

Attend weekly Single Point of Access Meetings (SPOA) participate in discussions in relation to in-reach referrals

Work collaboratively with other agencies including G4S to effectively manage individuals, information gathering and contributing to case conferences and ACCT reviews .

Foster good working relationships, to enhance joint working with other agencies to enhance release planning.

Care coordinate/co work individuals allocated to your caseload

Ensure record keeping meets the standard required by the NMC, IT systems as directed by the Team Leader and ensuring information sharing protocols are understood, and appropriately explained to clients.

Undertake comprehensive assessments using the Mental health measures framework, looking at strengths needs and risks (WARRN) .

You will be required to undertake a 5 day course ran by G4S prior to being allocated Keys to independently access the prison.

Job responsibilities

This post is fixed term/secondment for 8 months due to maternity leave. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesnt mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isnt fluency, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.

Key to the success of the post will be the effective working and partnership arrangements. They will be representative at relevant groups, meetings, forums and to provide specialist advice, knowledge on the management of mentally disordered offenders.

You may be required at times to provide training to G4S staff around mental health disorders and management of behaviours.

Person Specification


Registration

* UK NMC Registered Mental Health Nurse


Experience

* Experience of working within a multi-disciplinary team, and/or collaboratively with other organisations/agencies.
* Substantial post-registration experience in adult mental health care. Clinically credible with a background in multi-disciplinary working. Experience of managing/supervising staff.
* Experience of working autonomously and using own initiative
* Experience of working in NHS Primary Care Services/mental health settings/care homes/in-patient services.


Knowledge

* Professional knowledge acquired through Degree supplemented by post graduate Diploma specialist training. Evidence of continued professional development.
* Good understanding of risk assessment


Aptitude and Skills

* To work well within a team and foster good working relationships
* Good understanding of risk assessment
* Ability to work in a prison setting
* Strong and adaptable communication skills to suit varying environments and situations (written and verbal)
* Report writing and presentation skills. Change management skills
* Competent in assessment of risk
* Welsh language Speaking/Listening Skills Level 3/B2 or above
