About the Job
To provide holistic and person-centred care and support by telephone for patients with complex, life-limiting illnesses. The postholder will work within the multi-disciplinary team (MDT) and alongside registered nursing staff to contribute to the delivery of high-quality, evidence-based specialist palliative care.
Key Result Areas / Principal Duties and Responsibilities
* Act as the first point of telephone contact for patients, carers, and healthcare professionals, ensuring timely responses to requests for information or support.
* Liaise with Hospiscare’s Clinical Nurse Specialist teams to ensure a consistent and effective triage process, prioritising patients appropriately.
* Assist in identifying appropriate palliative and end-of-life care interventions to ensure safe, effective, and compassionate support for patients and their families.
* Communicate in a sensitive, empathetic, and professional manner with patients, relatives, and colleagues—maintaining dignity, respect, and Hospiscare’s core values.
* Adhere to Hospiscare’s Clinical Recordkeeping Policy, ensuring patient confidentiality when providing or sharing verbal, written, or digital information.
* Attend and contribute to multi-disciplinary team meetings and case discussions to ensure coordinated, person-centred care.
* Assist in identifying and assessing complex bereavement needs, ensuring seamless referrals to Hospiscare’s Supportive Care Team.
Infection Prevention and Control
Maintaining a safe and hygienic environment for patients, visitors, and staff is everyone’s responsibility.
* Adhere to Hospiscare’s Infection Prevention and Control Policy at all times.
* Ensure personal and team compliance with infection control standards.
* Report or act on any identified risks promptly to maintain a safe environment.
Disclosure and Barring Service (DBS)
This role requires a Disclosure and Barring Service Enhanced level check with the Adults Barred List.
