Job Description: Community Neuro Rehabilitation Team Member
We are seeking a dedicated professional to join our multi-disciplinary Community Neuro Rehabilitation Team (CNRT) to support patients with stroke or other neurological conditions in Torbay and Teignbridge.
1. Team Contribution: Be a core member of the CNRT, contributing to effective assessment and discharge planning of patients.
2. Assessment and Referral: Conduct conversations to identify patient needs and risks, and gather accurate information for referrals to the multidisciplinary team or other community services.
3. Post-Discharge Support: Under remote supervision, provide support after hospital discharge, including case management for those on the discharge to assess (D2A) pathway, recognizing changes in care needs, offering carer support, financial advice, community access, and holistic social care assessments. Prepare care plans, commission services, and ensure care packages are beneficial, cost-effective, and compliant with statutory requirements.
This role offers an exciting opportunity to work across Torbay and Southern Devon NHS Foundation Trust, and Devon County Council. You will undertake needs-led assessments in partnership with service users, families, and carers, managing caseloads to ensure effective care packages, all while adhering to policies on equality and diversity.
