An exciting opportunity has arisen for an individual to join our fast-growing, friendly team as an Advanced Nurse Practitioner for 22.5 hours per week.

We offer 6 weeks annual leave plus 8 days Bank Holiday (pro rata), along with NHS Pension and free, on-site parking.
Main duties of the job
The post holder will demonstrate critical thinking and be a committed, experienced nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
They will demonstrate safe, competent clinical decision-making and expert care for our patients.
The post holder will communicate effectively and work collaboratively with the general practice team and with our colleagues within the Integrated Care Centre to meet the needs of patients, supporting the delivery of policy and procedures.
About us
Cardigan Health Centre has approximately 10,400 patients. We are a GP training practice located within the new Cardigan Integrated Care Centre. We strive to deliver the highest quality healthcare to our patients through continuous training and development of all staff.
We are a short walk from Cardigan town and only a few miles from many beaches!
We work alongside other service providers in the Integrated Care Centre, including MIU, X-ray, Physiotherapy, Sexual Health, Outpatients, and District Nurses.
Our team comprises 4 GP Partners, 2 Salaried GPs, 3 ANPs, 3 Nurses, HCA, and a Phlebotomist, working alongside our large Reception and Admin team.
We currently use the Vision clinical system and are migrating to EMIS Web in July. Full training will be provided on both systems.
Job responsibilities
To deliver high-quality patient care as an Advanced Nurse Practitioner (ANP) in general practice, utilizing advanced autonomous clinical skills and a broad, in-depth theoretical knowledge base.
Working flexibly and collaboratively with GPs and other health professionals to provide comprehensive healthcare tailored to the needs of the local population.
Responding effectively to patient needs within professional boundaries, ensuring patient choice and accessible services.
Providing education and training to students and colleagues.
Supporting Practice Research studies as appropriate.
Contributing to the practice's development and sharing its values of excellence, responsibility, integrity, development, and commitment.
Providing a first point of contact for patients with undifferentiated problems, making use of skills in history taking, physical examination, problem-solving, and clinical decision-making to establish diagnoses and management plans, either face-to-face or via telephone.
Being accountable for own actions and working within own scope of practice at an advanced level using expert knowledge and clinical skills to deliver holistic care.
Initiating necessary diagnostic tests or investigations and interpreting findings/reports.
As an independent prescriber, ensuring safe, effective, and appropriate medication delivery following best practices and legislative frameworks.
Providing safe, evidence-based, individualized, holistic care, developing ongoing care/support plans emphasizing prevention and self-care.
Promoting health and well-being through health promotion, education, screening, and therapeutic communication skills.
Referring patients to other services or agencies as appropriate.
Collaborating with healthcare colleagues to ensure the delivery of National Service Frameworks (e.g., CHD, Older People, Diabetes, Mental Health, Asthma, COPD).
Working within the multidisciplinary team to promote integrated pathways of care.

Person Specification
Experience
* Minimum of 5 years post-registration experience, including recent primary care, community, or emergency setting experience.
* Evidence of CPD activities to maintain knowledge and competence in long-term condition management.
* Experience working autonomously and as part of a team.
* Ability to evaluate the safety and effectiveness of own clinical practice.
Knowledge
* Understanding of policy developments in primary care, including GMS, Clinical Governance, QOF.
* Knowledge of systems to assess the health needs of the practice population.
* Understanding of evidence-based practice and national standards (e.g., NSF, NICE guidelines).
* Awareness of accountability under the NMC Code and medico-legal aspects of the ANP role.
Qualifications
* Registered General Nurse (NMC registered).
* Recognized Advanced Practice qualification.
Skills & Ability
* Ability to assess and manage patient risk effectively and safely.
* Excellent communication skills, both verbal and written.
* Time management and workload prioritization skills.
* Ability to analyze data and draw implications for care plans.
* Effective communication within the multidisciplinary team.
* Welsh language skills are desirable.
