We have an exciting opportunity for a highly motivated and dedicated Chronic Disease Nurse to join our well established, forward thinking practice team in Rosyth.
Primrose Lane Medical Centre serves around 10,000 patients in urban West Fife from our purpose built health centre and are fortunate to have a fully staffed experienced multi-disciplinary team in our 6 partner GP Practice. We use EMIS and DOCMAN IT systems. The team prides itself in its friendly and supportive nature and meets regularly to catch up over tea/coffee to discuss matters both clinical and non-clinical.
You will be a fully qualified practice nurse with relevant qualifications in managing chronic diseases including, Diabetes, Asthma & COPD management, Hypertension and Cardiovascular Disease as well as general practice nurse experience. A non-medical prescriber qualification is desirable, but not essential.
You should have up-to-date knowledge and skills, coupled with a passion for high standards of professional care. We can offer training, support and encouragement to develop your role, including completing a non-medical prescriber qualification.
