Are you a Band 6 Occupational Therapist who would like to develop your clinical skills in a different area? Are you a Band 5 Occupational Therapist looking to develop your skills at a Band 6 level? If so, we have a brilliant opportunity to come and join us here in the District Therapy Team at Bolton Foundation Trust.
We work across six neighbourhoods in Bolton to help provide a holistic community-centred provision. This will link health and care services in areas so that it is easier for people to navigate services and get the care they need closer to home.
This includes GPs, therapists, community nurses, mental health practitioners, social care teams and Voluntary, Community and Social Enterprise sector providers.
Responsibilities
* Engage closely with acute staff, community colleagues and social care staff to coordinate and provide high quality care in the community to avoid hospital admission and support hospital discharges.
* Be part of a service that operates 7 days a week, 8am–6pm. Weekend working is on a rota basis, currently 1 in 6.
* Contribute as an experienced Occupational Therapist looking to develop in this clinical area.
* Utilise knowledge of community services to support patient care.
* Be an effective team player who is inspirational, flexible, adaptable and enthusiastic, and committed to developing patient-focused, clinically effective and efficient services.
* Play a key role in assessment, personalised care planning and management of care for patients who may require intensive home support as an alternative to hospital care.
* Participate in effective multi-disciplinary team meetings and work closely with the Local Authority.
Team and Environment
The role will be part of a multi-disciplinary team consisting of Physiotherapists, Occupational Therapists, Therapy Instructors, District Nurses, Pharmacy, Dieticians and Speech and Language Therapists.
