Position Overview:
My Client is seeking a skilled and compassionate Practice Nurse to join our dynamic healthcare team. The Practice Nurse will play a vital role in delivering patient-centered care within our practice. This individual will work collaboratively with other healthcare professionals to provide a wide range of nursing services, including health assessments, vaccinations, wound care, chronic disease management, and health education.
Key Responsibilities:
* Conducting health assessments and screenings, including blood pressure monitoring, BMI calculations, and health risk assessments.
* Administering vaccinations and immunizations according to national guidelines and practice protocols.
* Providing wound care and minor injury management, including dressing changes and suturing as necessary.
* Assisting with chronic disease management, such as diabetes, asthma, and hypertension, through regular monitoring, education, and support.
* Performing cervical cytology (smear) tests and other relevant screening procedures.
* Supporting the implementation of preventive care initiatives and health promotion programs within the practice.
* Maintaining accurate and up-to-date patient records using electronic health record systems.
* Collaborating with GPs, nurses, and other healthcare professionals to ensure coordinated patient care and effective communication.
* Participating in multidisciplinary team meetings and clinical audits to review and improve service delivery.
* Providing patient education and counseling on various health-related topics, including lifestyle modifications, medication adherence, and self-management strategies.
Qualifications and Experience:
* Registered Nurse qualification with current registration with the Nursing and Midwifery Council (NMC).
* Previous experience working as a Practice Nurse or in a similar primary care setting is highly desirable.
* Proficiency in clinical nursing skills, including health assessments, wound care, and vaccinations.
* Knowledge of chronic disease management protocols and evidence-based practice guidelines.
* Excellent communication and interpersonal skills, with the ability to engage effectively with patients from diverse backgrounds.
* Strong organizational skills and the ability to prioritize tasks in a fast-paced clinical environment.
