What You Will Do:
Conduct assessments and reviews of adults in need, ensuring compliance with the Care Act 2014.
Develop personalised support plans in collaboration with adults, carers, and other professionals.
Undertake safeguarding adults’ work to protect vulnerable individuals and uphold their rights.
Actively support the Duty system, responding to urgent cases and prioritising workloads effectively.
Maintain accurate records and track progress using MOSAIC, ensuring timely interventions and quality reporting.
