An exciting opportunity has arisen for a Health Inequalities Care Navigator.
12-month Secondment opportunity
Transforming care is a key commitment set out in the NHS Long Term Plan and is key to achieving the aims of personalising outpatients service. This means giving patients and their carers more control and greater choice over how and when they access care.
This role will support the delivery of personalised care plan within outpatient services. A personalised outpatient model can then provide patients with care that is better suited to their unique needs and circumstances, given through conventional means where necessary, but also empowering supported self-management and patient activation, enabling choice and shared decision making, social prescribing and community based support.
The post holder will support individuals to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them. This role will support people to develop their knowledge, skills and confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health related behaviours.
Main duties of the job
The successful candidate will be expected to support the reduction of DNAs (Did Not Attend) within various departments within our outpatient services and provide relevant support to patients as necessary including social prescribing and community-based supports.

Knowledge & Skills
Essential criteria
1. Excellent communication and interpersonal skills
2. Awareness of NHS long term plan
3. Knowledge of the comprehensive model for personalised care.
4. Computer literate able to use power point, excel and a basic knowledge SharePoint and experience/knowledge of information gathering systems and be able to analyse and report basic care data.
5. Excellent planning, prioritisation and organisational skills
6. Efficient and accurate word processing skills
Qualifications
Essential criteria
7. GCSE in Maths and English (A-C/4-9) or Functional Skills (level 2)
8. NVQ3 level Secretarial or Business Administrative qualification, or equivalent level of demonstrable experience (using the knowledge and skills criteria above)

9. Experience of clerical/administrative work involving data handling and management
10. Experience in the use of Electronic Medical Records in retrieving patient information
11. Experience of working within a healthcare environment
