Main duties of the job
You will be part of a multidisciplinary team, receiving clinical and managerial support, working alongside social care professionals, aligned with the NHS 10-year plan. Your responsibilities include delivering quality care to a designated caseload in patients' homes, following care plans devised by community nursing staff, and providing concise feedback during daily handovers to adapt to changing needs and escalate risks.

Responsibilities
The successful candidate must be flexible to cover service needs across the wider team, which includes working in various locations. Access to a car is essential. A full induction will be provided.
Location: Mid Bedfordshire (Ampthill, Shefford/Biggleswade)
Posts are available to work with the Biggleswade community nursing team, delivering care to residents of Biggleswade, Sandy, Potton, and surrounding villages, and the Ampthill team, covering Flitwick, Barton, Woburn, and nearby villages.
The service operates from 08:00 to 21:00, with shifts including weekends, evenings, and Bank Holidays. Part-time options are available.
Person Specification
Education/Qualifications
* Essential: NVQ Level 2 or significant care experience; willingness to work towards NVQ Level 3
* Desirable: NVQ Level 3, recent Care Certificate, GCSE or equivalent
Knowledge and Skills
* Essential: IT literacy, good verbal and written communication
* Desirable: Rehabilitation skills, data collection, delivering care in unpredictable settings, safeguarding, risk management
Other
* Essential: Manual handling skills, current driving license, ability to travel across sites, fluent English communication
Experience
* Essential: Experience in community health or social care
* Desirable: Team player, excellent communication skills
