Qualifications:
* Certified Nursing Assistant (CNA) license (required)
* 12 months of caregiving experience (required) (Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice)
* Current CPR certification (preferred)
* Hospice experience (preferred)
* Excellent customer service skills
* Strong communication (written, verbal, and active listening)
* Problem-solving skills
* Time management skills
* Ability to work in an electronic medical record system
* Ability to lift 50 lbs.
* Good understanding of body mechanics
* Strong understanding of professional boundaries
* Valid driver's license (required)
Responsibilities:
* Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
* Review patient care plans
* Assist with lifting and transferring patients
* Assist with range of motion, positioning, exercise, and ambulation
* Assist with dining and nutrition
* Maintain electronic medical records
* Work under the direction of a Case Manager RN
