Key Responsibilities
* The assessment of patients referred to or under the care of the North Warwickshire and Rugby CRHT, arranging all investigations and treatment, including regular review as specified by the Consultant.
* Medical care of patients with referral to or advice from the GP and/or other doctors where appropriate.
* Commencing and supervising treatment, including medication and other psychological treatment programmes as indicated and approved by the Consultant.
* Upkeep of clinical records, which are primarily electronic, reviewing notes and checking investigation results.
* Regular domiciliary visits to assess and review patients as required.
* Referring patients to other specialties and liaising with other agencies as appropriate.
* Preparing discharge letters or reports as required.
* Liaison with carers and family members as appropriate.
* Regular feedback to the Consultant and/or the MDT regarding problems and patient progress.
* Attending handovers, case conferences, safeguarding or planning meetings and multidisciplinary team meetings, and carrying out any other duties allocated by the Consultant, including follow‑up of patients.
