What you will do:
* Assess and implement tailored therapeutic plans for patients within the Intermediate Care setting.
* Facilitate timely hospital discharge and assist in the discharge-to-assess process.
* Collaborate with multidisciplinary teams to provide holistic patient care.
* Utilise your experience with SystmOne, if applicable, for efficient patient management.
* Travel to various sites and patients’ homes as required, so having a car is essential.
Responsibilities / General Duties:
* Bring your intermediate care experience to the role, ideally with hospital discharge or discharge-to-assess experiences.
* Utilise SystmOne for record-keeping and patient management, if you have prior experience.
* A full driving licence and access to a car are crucial due to the nature of the role requiring travel.
Required:
* HCPC registered.
* Driving licence with access to a car, as travel to various locations is part of the job.
