Job Duties:
* Undertake assessment of need and establish eligibility for services under Fairer Access to Care and the Care Programme
* To manage, monitor and review care plans involving service users and carers in liaison with the other workers and agencies as necessary to meet service user’s assessed need.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
