Overview
Lincolnshire Community Health Services are seeking a full‑time District Nurse / Case Manager to join the Lincoln North Community Nursing Team based at Ravendale Health Centre in the Lincoln North area. The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery. Extensive communication, interpersonal and clinical skills are required for success.
Responsibilities
The District Nurse/Case Manager is integral to leading patient care and to providing leadership for the community nursing team during challenging times and through the transformation of services. This role role‑models excellent behaviours in line with the trust values, and is essential to improving the quality of care delivered and ensuring a positive patient experience.
Qualifications
Applicants should hold the Community Specialist Practitioner qualification and have experience managing a caseload of patients and leading a team of staff to deliver excellent patient care. Alternatively, a Case Manager without the DNSPQ may be considered for the right candidate with a minimum of two years experience within a Community Nursing Team and the willingness to undertake the DNSPQ course.
