The Home First Intermediate Care Service supports patients to achieve their optimum independence by facilitating earlier discharge from hospital and avoiding hospital admission. You will assist in delivering high standards of health care and support patients within their own homes by following prescribed packages of care and therapy.
The vision of the service is to provide outstanding, patient-focused, community-based health care and therapy, optimising people's potential to achieve their goals by supporting earlier discharge from hospital, early assessment for care needs, and avoidance of hospital admission.
Our operating hours are from 7am to 10pm, seven days a week.
Please note: this post may be offered as a Band 2 Development post depending on qualifications and experience. You will be fully supported to develop into a Band 3.
Requirements: For this role, you will be required to travel independently around the county meeting strict time deadlines. You must hold a full UK driving licence and have access to a vehicle. Public transport cannot be used for this role as it is unreliable and will not meet service needs. Please confirm in your application that you meet these criteria.
Main Duties:
1. With training, use skills from professional areas such as Nursing, Allied Health Professionals, and Medicines Management to undertake delegated tasks.
2. Apply competency-based treatment techniques/care to patients with specific conditions under supervision, following prescribed treatment/care plans.
3. Provide care to patients experiencing health or functional deterioration, focusing on admission avoidance to hospitals.
4. Deliver functional assessments, personal care, medication prompts, including mobility and exercise, personal and domestic activities of daily living, equipment provision.
5. Carry out duties delegated and supervised by registered professionals.
6. Provide a high standard of care.
7. Manage your own caseload of patients requiring ongoing management and support with complex needs.
