Responsibilities:
* Serve as a Duty Social Worker within an Integrated Learning Disability Team, collaborating across agencies to deliver seamless health and social care services.
* Accurately record actions and decisions using the Mosaic system.
* Provide advice, assistance, and preventative support to adults with learning disabilities.
* Respond promptly to urgent duty matters, ensuring safety and wellbeing.
* Conduct assessments, reviews, and emergency visits, maintaining a focus on safeguarding and risk management.
* Apply principles of the Mental Capacity Act appropriately in social work practice.
* Maintain high professional standards in social care, adhering to Social Work England standards.
