Description About the role Corserv Care is part of the Corserv Group and provides a range of services under one brand. As a Discharge Reablement coordinator you will work collaboratively with colleagues from different teams, disciplines and partner organisations in order to provide a seamless service to individuals, families and carers. Your job will be: Screen all initial new referrals into the “hub” to assess initial pathway and criteria of service Schedule and carry out planned visits to service users to produce initial reablement / support plans, including initial risks assessments, moving and handling risk assessment to minimise the areas of risk to service user, staff, service providers and the general public. Monitor and undertake reviews of the service provided to service users according to published Quality Standards. Work collaboratively with other services to offer and assess further support using a TEC enabled approach. To undertake the relevant training and be responsible for assessing for and providing low level pieces of equipment which will enable customers to remain safe and independent at home. Receive and investigate complaints, accidents and incidents according to procedure and ensure that any safeguarding issues are dealt with expeditiously Close liaison with the Corserv Care’s, Hospital Discharge Co-ordinator, based at RCHT to ensure capacity is prioritised for medically fit patients requiring discharge to Corserv Care, through either the STEPS or Care & Support Teams to provide packages of care. Close liaison with the Registered Managers and community based teams to discuss care needs of potential new customers to ensure they can be safety met by the team. Co-ordinate with community-based Teams to arrange follow on start dates, visits and care plan completion for new customers. To work as part of a rota pattern that covers the core hours of 8am – 6pm and includes working alternate weekends. We are looking for you to bring: Minimum Level 3 qualification in health, social care or relevant areas or the capacity and willingness to work towards appropriate qualifications Knowledge and experience of relevant legislation, policy and principles, including Social Care and Human Rights legislation and Person-Centred care policies and practices and applies in all day to day activities Demonstrates good skills and abilities of electronic programming/rota systems plus experience of Microsoft Office packages Experience in social care with adults in need of support services and their families or a demonstrable commitment to adult social care Ability to use effective interviewing skills to undertake assessments of need, including risks and strengths.
