Main duties of the job
The successful candidate will provide medicines management support and education to patients in their homes, including after their discharge from hospital. By assessing patients' ability to manage their medication and supporting them with tailored care plans, the post holder will help maintain people safe and independent in their own homes. Other duties will include liaising with GPs, community pharmacies, and care providers to ensure safe and effective medication use. By collaborating with social care partners, including the delivery of medication training to care workers, the successful candidate will support the integration of health and social care. Working independently but within a supportive team, the role will utilise clinical skills and evidence‑based approaches to improve patient outcomes.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short‑listing and selecting candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.

Role Requirements
* Pharmacy Technician registered with the GPhC.
* BTEC Pharmacy Technician, NVQ Level 3 in Pharmaceutical Sciences or equivalent.
* Strong knowledge of medicines management across care settings.
* Experience working in multi‑disciplinary teams and delivering patient‑centred care.
* Excellent communication, organisational, and problem‑solving skills.
* Ability to travel across the UHB locality.
* Welsh language requirements: Welsh Level 1 (or readiness towards it).
Person Specification
Qualifications and Knowledge
* BTEC Pharmacy Technician, NVQ Level 3 in Pharmaceutical Sciences or equivalent.
* Pharmacy Technician registered with the GPhC.
* Knowledge of primary/secondary care medicines management issues.
* Knowledge of available systems to support patients.
* IQT knowledge / qualification.
Experience
* Experience working in multi‑disciplinary teams.
* Experience of delivering near‑patient pharmacy services.
* Experience of service/quality improvement.
* Experience of conducting medicines needs assessments.
* Welsh language speaking/listening skills at a Level 3 or above.
* Experience of working with social care services / providers.
