Job summary
Specialist Medicine care group requires an enthusiastic, dynamic, and highly motivated care pathway facilitator to assist with discharges and flow across the care group. You will be responsible for paperwork according to the pathways relevant to each discharge, early planning of discharges from admission, and demonstrating excellent communication skills. Confidence to challenge in difficult situations and cross-cover at KGH if needed are essential. The post is currently Monday to Friday, but this may change in the future. You will work as part of the multidisciplinary team (MDT) and collaborate with external professionals to ensure the safest discharges for our patients.
Main duties of the job
This role involves joining an existing team of care pathway facilitators to plan and manage timely and appropriate transfers of care (discharges) for patients from the hospital. The focus is on supporting the Trust's goal to increase discharges before 10 am and to facilitate same-day discharges. You will work closely with ward teams to identify potential discharge issues early, communicate with patients and families/carers on admission, and coordinate with multidisciplinary teams and system partners to ensure smooth and safe discharges. The role requires ensuring discharge plans are well communicated and in place for patients no longer meeting the criteria to reside in an acute hospital bed.
