An opportunity has arisen to join the End of Life Care team as a Band 3 Healthcare Assistant on a part-time (13 hours/week), permanent basis.
The successful candidate would be welcomed into an established team working closely with members of the wider multidisciplinary team across all the PCNs in Surrey Downs Health and Care.

We are seeking a Healthcare Support Worker to work within the End of Life Care Team. In this role, you will provide care and support to palliative patients within the community setting, ensuring a seamless service as a complete package of care is put in place. The post holder will undertake a range of delegated tasks and will be supported to develop skills and abilities to work with this client group to provide appropriate care.
You should have excellent organisational and time management skills and be passionate about delivering holistic, patient-centered care. You will be encouraged to develop new ways of working to improve quality and foster a "one team" culture.
