Job Summary
Working Part time: 5 Sessions per week
Salaried GP with a number of combinations of sessions offered. The Hospital at Home service provides monitoring and treatment at home, including intravenous (IV) therapy, with related components such as Virtual Wards, Urgent Community Response (UCR) and IV OPAT. The post holder will undertake medical management including clinical assessment to ensure safe management of patients within the community, working with primary and secondary care colleagues to support admission avoidance and early discharge where clinically appropriate.
As pressure on emergency departments grows, new care models are pivotal in delivering care away from hospitals and into the community. This role involves working collaboratively with other multi-disciplinary teams within primary and secondary care and developing leadership skills in evolving primary care roles.
Hospital at Home (Clinical sessions-negotiable).
Main duties / Responsibilities
* Manage the medical requirements and provide medical oversight to patients being cared for by the intermediate care service (Hospital At Home).
* Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral to other agencies where appropriate.
* Develop, monitor and adjust clinical management plans for health in consultation with patients and other team members and in line with current pathways and protocols.
* Liaise with secondary care consultants in the management of patients within the intermediate care pathways.
* Undertake and order appropriate clinical diagnostic testing including arterial blood gas analysis, X-rays and blood tests.
* Be competent in arterial blood gas interpretation and ECG interpretation.
* Assist in developing appropriate pathways for people in intermediate care settings and for people being cared for by the team to avoid unnecessary hospital admission.
* Assist in developing appropriate pathways for people in secondary care settings to facilitate safe early discharge into the intermediate care service.
* Work in accordance with the service rota, undertake clinical sessions including telephone advice and visiting patients at home; weekdays and weekend GP on-call rota.

Qualifications
Essential
* A medical practitioner listed on the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003
* Not subject to suspension under section 41A of the Medical Act 1983
* Qualified General Practitioner (CCT)
* Currently on a CCG performers list and not suspended from that list or the medical register
* Annual NHS appraisal
Desirable
* Evidence of further postgraduate educational activities in relevant fields
* MRCGP
* DCH
* F2 supervisor training/teaching diploma (essential for HDS)
* Evidence of CPD activities
