The service has a proven track record in providing effective services to individuals with a history of trauma, self-harm and interpersonal difficulties. The service has now been adapted to better meet the needs of young people aged 18-25. EMERGE Leeds provides a range of interventions for people with complex emotional and relational difficulties including care coordination, Youth Work, Housing, Peer Support, Occupational Therapy, Psychotherapy and Creative Therapies. Senior Mental Health Practitioners sit at the heart of our established service providing case management and care coordination in the context of the multi-disciplinary team.
Main duties of the job
Senior Mental Health Practitioners in the service manage a caseload and provide care coordination as part of a psychologically informed service, alongside colleagues from range of disciplines and partner organisations. The successful applicant will be expected to develop positive and meaningful therapeutic relationships with individuals who experience emotional distress and interpersonal difficulties, and to provide effective interventions and case management.

About Us
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. Team members are provided with training and access to both regular clinical and managerial supervision.
