An exciting opportunity has arisen for a highlymotivated professional to join our Hospice Community Nurse Specialist team.
Main duties of the job
We are looking for a nurse who is passionate andunderstands the needs of those receiving palliative - end of life care and have aworking knowledge of evidence-based practice. You must be able to practiceautonomously and independently as well as be an effective member of themulti-professional team, working collaboratively with our health careprofessionals. You will therefore have excellent communication skills. Acommitment to the progressive development of the community specialist nurserole is also essential.
We will consider applications for anyone wishingto work between 30 37.5 hours per week over 4 -5 days Monday to Friday.

CLINICAL RESPONSIBILITIES
To be conversant with thepolicies and procedures for Weston Hospicecare
To be responsible inidentifying, prioritising and implementing initial and ongoing assessments ofthe physical, mental, emotional, spiritual, social and practical needs of thepatients and their families carers
To establish and maintaingood professional relationships with members of the Primary Health Care Teamproviding knowledge and expertise in the care of patients with incurabledisease. This includes advice on pain and symptom management, social, psychologicalcare and communication
To build and maintain acontinuing caring therapeutic relationship to enable the patient and theirfamily carer opportunities to discuss problems, anxieties, fears, hopes andjoys.
To offer advice and supportto meet the physical, psychological, social and spiritual needs of patients andtheir families carers
To liaise with the hospicedoctors on issues relating to patient care
To maintain accurate patientrecords in accordance with the NMC Code of Practice
To undertake administrationand recording of drugs in accordance with Hospice policy
Report, record and deal withall accidents, incidents and complaints in line with Hospice policy
To evaluate theeffectiveness of nursing interventions and treatments
To take responsibility forthe management of own patient caseload & assist in the planning,implementing, evaluation and documentation of direct patient care to meetagreed clinical objectives and standards of care

Special Knowledge
* Understanding complex symptom management in specialist palliative care

Qualifications
* First level RGN with minimum 5 years experience
* Relevant degree or working towards
* Current NMC registration
* Evidence of continuing professional development
* Teaching qualification communication skills module
* BSc in Palliative Care or relevant social science

* Able to transport self within community catchment area
Communication and people skills
* Good communication skills including verbal, non-verbal, written, electronic and presentation. Communication will be mostly in a clinical environment with other professionals, patients, cares & public, including out of hours advice line

* Basic PC skills including email and use of patient software system
Experience
* At least 5 years at senior nurse level, at least 2 in specialist palliative care or community
