The Role
The post holder will have responsibility for assisting in the early identification of people who require assessment of care needs to facilitate effective discharge from hospital. The post holder will ensure that there is effective communication between all discharge support services internal and external to Health, to support the delivery of high quality holistic discharge planning to people currently receiving care in hospital and their carers. particularly those who will require additional support from community based services on discharge. The post holder will play a lead role in the education of ward staff around the completion of referral information to a range of community based services.
The post holder will assume responsibility for implementing joint health/social work discharge planning standards and give specialist advice, support and education to staff within the Hospital and multi-agency team on the discharge planning process.
The post holder will report to the integrated service manager and will provide leadership related to discharge co-ordination, including the development and practice of staff within all areas of the hospital.
The post holder will liaise with ward managers re the training needs of their staff in terms of providing good quality information to community based services that lead to effective discharge of people from acute care.
The post holder will be required to have good knowledge around the range of community based services that are available to support people to live and die in their own homes and be able to impart this to the range of staff within acute care.
The post holder will be responsible for generating and maintaining information regarding the status and numbers of people currently receiving care in hospital who are ready for discharge and liaise with other discharge support service managers regarding discharge arrangements for this group of people. This will involve extensive communication with Housing Social Work services, Hospital ward staff. Community and Rehabilitation Teams and Community Nursing services.

What we'll need you to bring
* Registered Nurse or AHP with valid NMC / HCPC Registration.
* Educated to/working towards/operating at Degree Level.
* Post graduate expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
* Comprehensive knowledge of clinical guidelines and standards within the speciality / area of practice.
* Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality / area of practice.
* Management qualification or equivalent gained through experience and continuous professional development.
* Further education/evidence of Continuous Professional Development in area of speciality including study days/courses/post graduate qualification.
* The post holder will possess excellent team-working/leadership skills and have the ability to motivate others and work using own initiative.
* Excellent listening, communication and interpersonal skills.
* Effective time management skills.
* Working knowledge of basic information technology.

This role will be based in Frailty within University Hospital Wishaw.
